The Divine HealthCare team works together to ensure the best quality home care for all of our patients. All of Divine HealthCare professionals meet our rigorous employee requirements and have graduated from an accredited school for their specialization. Additionally, each skilled employee receives an extensive orientation with Divine HealthCare’s operational policies and procedures. All disciplines must complete at least 12 continuing education hours each year and attend company in-services to increase their knowledge base and to keep current in their respective fields. Finally, each discipline adheres to a code of ethics and accountability.
Responsible for planning, coordination, management, and direction of all activities and programs of the agency.
Oversees all aspects of quality control within the agency. Works with the Clinical Supervisor to identify areas of improvement for the agency staff.
Responsible for the overall direction of home health clinical services. Establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy.
Responsible for care coordination and case management. Assures that care and services are delivered to meet patient needs and are according to physician orders.
Responsible for the implementation of standards of care for skilled services as prescribed by the physician and in compliance with applicable laws and regulations and policies of the agency. Completes the initial assessment visit and coordinates the service the patient receives from admission/referral to discharge.
