Divine HealthCare Network (DHCN) is a fully bonded, insured, and Medicare- and Medicaid-certified home health care agency with a comprehensive Minnesotan license.

We have been in business since 1996, providing skilled nursing, personal care assistance (PCA), home health aide (HHA), homemaking (HMK), respite, home care nursing (formerly private duty nursing), therapy services, and complex/specialty health services. We strive to meet the needs of our clients, allowing them to remain independent in their homes.
